This analysis is not meant to be thorough or scientific but merely to assist you to understand where your strengths and weaknesses may be at this time in your ministry.

____ 2. Is there a follow up system or a way of following up visitors that works and do you have a follow-up team devoted to this?
____ 3. Do you have a clear system for helping people move from the crowd to the congregation to the committed; eg, offer classes for development like 101, 201, 301, 401?
____ 4. Are expanding your ministry teams for Worship, Kids ministry, Assimilation, Small Groups, and other teams as needed?
____ 5. Do you have at least 50% of your adult attenders in small groups i.e. affinity, task, support or bible study groups?
____ 6. Have at least 25% of your adult attenders gone through your membership classes?
____ 7. Are you regularly sharing stories about people who have come to Christ or recommitted their lives?

____ 2. Are your members giving regularly and sacrificially to your church with time/talent/treasure?
____ 3. Has your TLT – Transitional Leadership Team – begun to prepare the church for the Leadership Team; or, has your Leadership Team begun to function as a group of strategic, spiritual leaders?
____ 4. Are your ministry teams expanding in number the number of people on each team so at least 25% of your adult attenders are serving in some way?
____ 5. Are you having quarterly leadership meetings for all your servant-leaders for VHS & fun (Vision, Huddle as a team, and Skill development & fun together, fun thanking people for their contributions, etc)?

18 to 20 checks This reveals that you may be in excellent health with a steady ministry momentum (Green Flag)
15 to 17 checks This is OK but exercise caution and review where your ministry is heading (Yellow Flag)
12 to 14 checks This may reveal stagnation may be slowly settling in – review this with your leadership (Red Flag)
11 and under Sounds like you may be in need of ministry intervention by your coach asap.
